Unplanned work (breakdown) reporting is an integral part of any CMMS. Some systems allow end users (production personnel or asset user) to access an input screen, through which they can report defects or breakdowns. Others require  a maintenance controller to raise the defect report after having been notified by the end user. Traditionally, the process is as follows:-

Breakdown occurs 
End user reports fault to maintenance supervisor 
Supervisor enters breakdown details into CMMS and generates a work order 
CMMS changes asset state to "not available" and maintenance task to "pending"
Trades person completes maintenance job and reports to supervisor
Supervisor enters failure resolution information into CMMS
Asset state changes to "available" and maintenance task "completed"
CMMS computes Availability & MTBF based on human generated inputs
